Eldest sons are referred to by their father's next highest unused title. (For example, John Howard, Duke of Norfolk, is also the Earl of Surrey, but since Dukedoms are higher than Earldoms, is referred to by the Duke title, and his son Thomas Howard goes by the title of Earl of Surrey.)
